Cordata Haven Care Coordination Platform | Onondaga County, NY

Haven is Cordata Healthcare Innovations' care coordination solution for addiction and mental health. Cordata describes it as a way for agencies to organize follow-through for people who need support after an overdose, a behavioral health crisis, a public safety encounter, an outreach contact, or a referral from a partner. Participating organizations share visibility into client needs, outreach activity, referrals, assessments, services, and follow-up. Who might use it

County teams that coordinate a post-overdose or post-crisis response across more than one agency, including sheriff's offices, overdose response teams, quick response teams, and mobile crisis teams. The behavioral health providers, peer programs, hospitals, and social service agencies that send and receive referrals inside that response are the other half of the audience, since a coordination platform only works if the partners use it.
